[dev] Horde_Form patch

- clean up of the Horde_Form when handling file uploads
- added an extra parameter "enctype" to the open() function, should i be
defaulting to null ("enctype = null") in the Renderer::open() function?
- added a checkdate control to the monthdayyear input validation to trap silly
dates.

someone please have a test of this thanks,
