Depends on your operating system and setup. If using unix, then I think
the easiest way to upgrade Horde is to use the patch sets found at

http://ftp.horde.org/pub/horde/patches/

However, there are two problems for you here.

1) There is no patch set for going to 2.2 yet (But I hope there will be soon)

2) The patch sets only go between sucessive versions, so you would need to
   do two patches: 2.0 -> 2.1 -> 2.2

Once you apply the patches, then you need to check all the configuration files
by comparing the *.conf and *.conf.dist and merging in any changes.

Then you need to look at docs/ for a file called UPGRADING (not applicable
to your upgrade, but I'll mention it anyway). If changes are needed during
an upgrade (for example to your sql, ldap, etc) they should be noted in
this file.

Correct. No one has submitted one, and it is dependent on things like
operating system, if update via RPM/tar/patches/CVS, etc.

Once the patch sets are out, it shouldn't be too hard. Otherwise it is
a bit more complicated, but not much.
