The preference hooks are only run the first time that the user logs into the
system, the result of which are stored in the user preferences.  To have
this preference value updated upon subsequent login, the preference value
must be locked ('locked' => 'true').

Having just gone through all of this myself with the help of Mike Cochrane,
this appears to be the only element which you are missing.
