No idea.

Do you have more web server children then allowed DB connections?  If so,
either lower the number of web server proccesses, increase the number of
DB connections, or disable persistent DB connections.

Nope.

Nope.  Mailing list archives would be the place to start for vhosting info,
in particular the IMP mailing list, then the Horde mailing list after that.
Your favorite search engine should help you locate some postings.
