[horde] clear username and password

    I did try <form method="post" action="/path" autocomplete="off">. I 
didn't work. I don't think it is browser problem. The values come from 
getenv('REMOTE_USER') and getenv('REMOTE_PASSWORD') in login.inc. They are 
filled in server side not client side.
