[horde] How can a user switch between DIMP and MIMP when authenticating against SQL server?

When using imap as authentication, there was a combo box on the login page where one could choose Traditional og Dynamic mode...
When authenticating against a SQL server, this combo box is gone. How can a user choose between these two modes now?
