Thanks Luis, I'll enable debugging logging and investigate. I'll also
try a another install of Horde on a different box and see if I receive
the same results. I'll get back once I have some more information.

Hello, Brad:
It's [another] new fresh install of your horde. I meant you should do
another installation in order to follow all the steps again, because
it's easy to forget to install some package needed or leave something
behind, and doing the process again helps to get it all.
The logout button is working fine here (both the sidebar and the top
menu).
You could also increase the log level on Administration > Horde >
Logging > PEAR_LOG_DEBUG and then hit the button and see if there's
something useful in the log file.
