Luis,
Good news! I found the problem that was causing the Log Out button in the sidebar to not work. The server with the Horde installation had PHP’s session.save_handler option set to “memcache” and had a local memcached server configured. Once I edited the php.ini file to set the session.save_hander to “files”, the problem went away.
I followed your advice and installed Horde from scratch on a test server I have in my office. It worked fine on the test server and I started to look for differences between the two. I’m using SUSE Linux Enterprise Server 10 SP1 so unfortunately the wiki article you suggested could only be used as a reference.
I suspect that the problem is somewhere in PHP and the memcache extension on the server. On my test server I could not install the memcache pecl module without disabling memcache session support during the install. I don’t know how memcache was installed on the production server so it could support memcache sessions. Maybe the installed did something to ignore the error.

Hello, Brad:
It's [another] new fresh install of your horde. I meant you should do another installation in order to follow all the steps again, because it's easy to forget to install some package needed or leave something behind, and doing the process again helps to get it all.
The logout button is working fine here (both the sidebar and the top menu).
You could also increase the log level on Administration > Horde > Logging > PEAR_LOG_DEBUG and then hit the button and see if there's something useful in the log file.
