Jan,

I have already tested the user/pass for the database and had redone a  
grant all privileges for that user and checked the mysql users tables  
for permissions.  Is there a way to make the script more verbose?  Can  
I run the DB section more manually to get the mysql errors?

I have had this problem with both the 4.x from a week ago and the new  
5.0.  Even did a pear uninstall on all the packages.
