[horde] Include javascript files to layout

Hi,

in order to include my JavaScript XMPP Client into Horde, I have to
include a couple of js and css files into your layout/template. I found
this pages, but have no clue, how to create an app out of this:

Maybe someone can give me a hint or a skeleton application.
