No idea if there is a freebsdish way to create a database.  But why not
just stick with the way your RDBMS sets up new databases and permissions
for those?

There is a select box on the login screen to chose from.  The chosen
interface is not a user preference in the Horde sense of the term. 
However, a cookie is set with the choice made so that returning users
will usually get the same type of interface as before.
