The latter one. I guess you didn't provide a consistent key (source name) 
in sources.php and attributes.php or left it empty completely in one of 
those.

No, but there are scripts in imp/scripts to migrate the user preferences to 
the new preference table.

Still can't reproduce it, even with magic_quotes_runtime On.
Must have been a local problem.
