[imp] LDAP on IMP 2.2.7

I didn't have the 1st "index" line and I did have the second one.  Adding 
the first had no effect -- but I didn't expect it to.  Again, I am able to 
query from the command line, but not from within IMP so I have to think 
there is a configuration within IMP not SLAPD that isn't correct.

My IMAP server is from a now-defunct company called Execmail (formerly 
Simeon).  I think their name du jour is now Messaging Direct.  It is based 
on the Cyrus Server.  IMP 2.2.3, 2.2.4 and 2.2.7 work(ed) fine.  I spent 
days trying to get 3.0 to work without success.
