[imp] Attachment Form Components Missing - RESOLVED

My second attempt to verify that the user wasn't simply failing to scroll
down (using smaller words and fewer technical terms) did confirm Eric's (and
my own) initial suspicions that the user was indeed failing to scroll down.

As penance for bringing a non-issue to the list I shall now stick around
until I can help someone else with an equally stupid problem. :)
