Not easy replying to a top reply but...

you need to execute it from php

[root at srv1 ~]# which php
/usr/bin/php
[root at srv1 ~]# /usr/bin/php -f \
/var/www/html/horde/kronolith/scripts/reminders.php
X-Powered-By: PHP/4.3.9
Content-Type: text/html; charset=ISO-8859-1

[root at srv1 ~]#

set it to a cron with a repeating interval.

# cat /var/spool/cron/apache
0,2,4,6,8,10,12,14,16,18,20,22,24,26,28,30,32,34,36,38,40,42,44,46,48,50,52,54,56,58 * * * * \
/usr/bin/php -f /var/www/html/horde/kronolith/scripts/reminders.php \
