[sam] Different users seting same address not working

Greetings,

I've black and white listing working now correctly, except for one thing:
my domain user foo at bar.com has put spammer at junk.com in blacklist. When another
user from my domain, bar at bar.com tries to put the same address spammer at junk.com
in his blacklist, he gets an error ( Error  Cannot set options: DB Error:
constraint violation ).
>From the logs:

Sorry about the brazilian portuguese localization.

Note: they share the same policy id. But AFAIK the email addresses in
black/white list don't have to do with it.
