[turba] Import Addresses from Korganizer

Basically, any fields that you would like to use in Turba, whether  
they are from an imported source or not, *must* be defined in  
attributes.php and sources.php so that turba knows about them. In  
addition, if you are using a SQL backend, you must also be sure to  
create fields in the database to hold the data. You can look at the  
various sql scripts for a place to start and read the sources.php and  
attributes.php files for information about the format of those entries.
