I wish I was getting some kind of logging action to help me out, but there
doesn't seem to be any indication anywhere that something is actually going
wrong. That's part of what makes this so frustrating, I don't know where to
start looking. For what it's worth, here's the relevant version information,
in case anyone happens to know of a "bad" version of something (again, I
really do appreciate all your suggestions):

I have similar problem with OpenBSD 3.6 on an Sparc64.  The difference 
is that I'm able to get test.php, and most of the things work.  However 
sidebar.php script makes Apache child process to crash.  The result is 
that left frame (where sidebar should be) is empty.  You may wish to 
check if your Apache server is also crashing.
