[horde] Error when upgrading databases from HGWE 1.2.11 to HGWE 5.1.1

Dear All,

I have started working on HGWE 5.1.1.  My previous set up is HGWE  
1.2.11.  All tables have got updated except

1. nag

Error is:

[  INFO  ] Current nag schema version: 9
Migrating to NagUpgradeCategoriesToTags (10)

QUERY FAILED: Duplicate entry 'XXXXXX at isac.gov.in' for key  
'rampage_users_user_name'

INSERT INTO `rampage_users` (user_name) VALUES ('XXXXXX at isac.gov.in')

2. horde_history

Error is:

[  INFO  ] Current Horde_History schema version: 4
Migrating to HordeHistoryRemoveCompositeIndex (5)
== 5 HordeHistoryRemoveCompositeIndex: migrating

-- removeIndex('horde_histories', 'history_modseq')
QUERY FAILED: Can't DROP 'index_horde_histories_on_history_modseq';  
check that column/key exists

DROP INDEX `index_horde_histories_on_history_modseq` ON `horde_histories`


Please guide to resolve these two issues.
