Double Entries in Log files...

Can anyone tell me why i'm getting a double login on some users in my auth logs...?? Also i'm not getting any logout confirmations?

After confirming with the users they didn't notice anything wrong with there 
session at all, but some users complain about getting logged out once they log in (php session problem? What to check/twig? Using oracle8 for sessions handling)
