Ok...we had a similar problem...we could not see ispell anywhere. Only
when we killed the offending apache process did ispell show up.

After that it was easy to reproduce...all we had to do was spell check a
10 page document pasted into an email.

If that is not your problem you may want to look at StartServers and
MaxSpareServers in your httpd.conf file.
